How to Apply or Get Help
Call the Foster Care Medical Team (FCMT) at:
1-800-562-3022, ext. 15480
(Don’t dial the extension right away — follow these steps):

Call the number and stay on the line.

When asked if you have an extension, say “Dial.”

When asked “client, provider, or other,” DO NOT answer—just press 3.
-
When prompted, enter extension 15480.
They also offer interpreter services if you need help in another language.
Lost Your AHCC or ProviderOne Card?

For AHCC card: Call 1-844-354-9876 or visit coordinatedcarehealth.com to get a new one anytime.

For ProviderOne card or other questions, call the Foster Care Medical Team using the steps above.
Choosing Your Coverage (Ages 18–26)

Fee for Service: You find your own doctors and manage your care.

AHCC Managed Care: Care managers help you find doctors and coordinate your physical, behavioral, and dental care.

Who can use this resource (eligibility)?
You might be eligible for medical coverage if you say YES to one of these:

You are under 21 and currently in foster care or another eligible out-of-home placement with Washington state or a recognized tribe.

You left foster care or eligible placement on or after your 18th birthday and are under 26 years old.
What Counts as Eligible Out-of-Home Placement?

Licensed foster care or relative care
-
Tribal foster care
-
Interstate Compact for Placement of Children (ICPC) foster care in Washington
-
Dependency guardianships started before June 2010
-
Other suitable court-ordered placements (even if unlicensed)
Adoption Support Program Youth

Eligible until 18 years old, or
-
Up to 21 years old if you’re still in high school
Important Note for Youth Moving from Another State

If you aged out of foster care in another state and then move to Washington, you might not qualify for Medicaid under the Health Care to 26 rule.

You can still apply for Medicaid based on your income.
