What Care Coordinators Do

Each youth and family is matched with a Care Coordinator who helps create a personalized plan of support. Here's how they help:

Build a Custom Plan

  • Work with families to identify strengths and needs

  • Develop a plan tailored to the youth’s goals and situation

Create a Support Team

  • Connect families with community supports and health care resources

  • Help build a reliable team of professionals and natural supports

Track Progress

  • Monitor how the plan is going

  • Make changes as needed to support success

Stay in Touch

  • Maintain regular face-to-face meetings and phone contact

  • Be there consistently for ongoing support

Provide Crisis Support

  • Offer help during tough moments or unexpected challenges

 Educate & Empower

  • Encourage youth and caregivers

  • Share knowledge and tools for long-term success

How Youth and Families Connect with Choices

Youth and families come to Choices through several referral pathways, including:

  • State and county agencies we partner with

  • Schools

  • Managed care organizations

Once referred, families are enrolled in our program and connected with a dedicated Care Coordinator.
